Muscle: One of the Most Important Tissues We Have for Healthy Ageing
Muscle is often associated with athletic performance.
It is discussed in the context of strength training, sporting achievement or body composition, yet its importance extends well beyond movement.
Skeletal muscle is one of the body's largest and most metabolically active organs. It influences glucose regulation, energy metabolism, physical function, balance and recovery from illness. As we grow older, it also becomes one of the strongest indicators of how well we continue to function.
For that reason, preserving muscle deserves to be considered alongside other aspects of healthy ageing, including cardiovascular health, nutrition, sleep and skin health.
Ageing Changes Muscle Long Before Frailty Becomes Obvious
Most people associate muscle loss with advanced age, but the biological process begins much earlier.
From around the fourth decade of our lives, muscle mass and strength gradually decline. This process, known as sarcopenia, progresses slowly over many years and varies considerably between individuals.
Physical inactivity, illness, inadequate protein intake, hormonal changes and chronic disease can all influence the rate at which muscle is lost.
Because these changes develop gradually, they often go unnoticed until they begin to affect everyday function.
Standing from a chair becomes more difficult. Carrying groceries requires greater effort. Recovery after illness or injury becomes slower than it once was.
\The gradual nature of these changes is precisely what makes prevention worthwhile.
Muscle and Longevity
One of the most interesting developments in ageing research has been the recognition that muscle provides information extending far beyond physical strength.
In 2014, Srikanthan and Karlamangla examined body composition in older adults and found that individuals with greater muscle mass experienced lower all-cause mortality than those with lower muscle mass, even after accounting for body mass index and other health factors.
Subsequent studies have reached similar conclusions using different measures. Grip strength, for example, has consistently been associated with future health outcomes. The PURE study, involving more than 140,000 adults across multiple countries, found that reduced grip strength predicted cardiovascular disease and all-cause mortality more strongly than systolic blood pressure in some analyses.
These findings do not suggest that building muscle guarantees longevity, but they might suggest that muscle reflects something important about biological resilience.
Healthy muscle supports movement, metabolic health, independence and the capacity to recover from physiological stress. It also reflects the cumulative effects of nutrition, physical activity, chronic disease and general health over many years.
What on Earth Does This Have to Do With Facial Ageing?
At first glance, very little!
The muscles that determine grip strength are clearly different from those involved in facial ageing. The connection lies elsewhere.
Ageing affects tissues throughout the body rather than in isolation. The biological processes that influence muscle (including chronic inflammation, hormonal changes, mitochondrial function, nutrition and reduced physical activity) also influence skin, connective tissue and repair mechanisms.
Facial ageing becomes one visible feature of systemic ageing rather than an isolated cosmetic problem. Supporting the health of the body as a whole creates the conditions in which skin, connective tissue and recovery can function more effectively.
Resistance Exercise Changes More Than Strength
Resistance training is often promoted because it increases muscle size, but its effects are considerably broader.
Regular resistance exercise improves muscle quality, supports bone health, enhances insulin sensitivity and helps maintain physical function throughout adult life. It also becomes increasingly valuable during periods of weight loss.
As we discussed in the GLP-1 section of the Resource Centre, maintaining muscle while reducing excess body fat has become an important consideration in modern weight management. Weight reduction achieved at the expense of significant muscle loss may influence long-term health, recovery and physical resilience.
For many adults, resistance exercise is therefore not simply part of a fitness program, it’s one of the most practical strategies available for supporting healthy ageing.
Nutrition Provides the Building Blocks
Muscle is continually being broken down and rebuilt, and maintaining that process depends on adequate nutrition, particularly sufficient dietary protein.
Ageing changes the way muscle responds to dietary protein, meaning older adults often require a greater stimulus to maintain normal muscle protein synthesis. This concept, known as anabolic resistance, has become an important area of ageing research and has influenced nutritional recommendations for older adults.
Nutrition should never be viewed in isolation. Physical activity, sleep, illness, medications and metabolic health all influence how effectively muscle adapts over time.
Looking Beyond Appearance
Muscle rarely receives much attention in discussions about facial ageing, maybe because it sits outside the traditional boundaries of cosmetic medicine.
Yet one of the most valuable lessons emerging from healthy ageing research is that the body does not separate itself into cosmetic and non-cosmetic systems.
The same biology that influences strength, mobility and recovery also influences tissue repair, inflammation and skin health. For that reason, preserving muscle should not be viewed as preparation for sport or a pursuit reserved for younger adults. It is one of the foundations that supports healthy ageing across multiple organ systems.
When I discuss prevention with patients, resistance exercise, adequate protein intake and maintaining physical function are not separate conversations from skin health or facial ageing - they are different expressions of the same principle: looking after the tissues that will continue supporting us in the decades ahead.
